    Arctic sea ice and snow cover albedo variability and trends during the last three decades

    The aim of the present study is to assess the full effect on the albedo of both sea ice extent decrease and snowline retreat in the Arctic during the last three decades. Averaged over the globe, the overall warming effect due to Arctic land and ocean albedo change corresponds to adding about 44% to the direct effect of human CO2 emissions during the same period. In fact, the area and thickness of Arctic sea ice have both been declining in this time frame. This has caused feedbacks affecting the whole global climate system. One such is albedo feedback of sea ice shrinking which was previously estimated (Pistone et al., 2014) to add about 25% to the direct warming effect of anthropogenic CO2 emissions. In this study, we demonstrate that the role of snowline retreat in albedo decrease is comparable to that of sea ice shrinking. To this aim, we estimate the radiative forcing (W/m2) due to snow and ice decrease during 34 years (1982–2015) from the analysis of changes of observed albedo based on the Clouds and the Earth's Radiant Energy System Energy Balanced And Filled (CERES EBAF) dataset, paired with sea ice and snow cover data from the US National Snow & Ice Data Center (NSIDC)

    The main critical issues of the gym environment in an Italian city

    BACKGROUND: The lack of a consistent national regulation regarding gym facilities, combined with the growth and transformation of the world of fitness, has led to an uncontrolled situation, where, especially in metropolitan areas, low cost gyms are continuously popping up, often not respecting the structural and hygienic requirements. AIM OF THE STUDY: Objective of this study is to evaluate the results of a monitoring programme about the gym environment, to highlight the main critical issues. METHODS: In 2018 a randomized sample of 90 gyms was inspected in Milan, using a checklist with three sections of inquiry and the resulting data were analysed through a series of multivariate regression models. RESULTS: As per the various aspects analysed, many outcomes with low scores concerned franchised gyms, which have shown to be unsatisfactory in many respects; in addition, the lack of L. pneumophila risk containment procedures has been observed in the facilities without a swimming pool, compared to those with it. CONCLUSIONS: The study results offer a clear picture of the gym environment, identifying many inadequacies for different hygienic and safety aspects; therefore, it has been possible to understand which issues need particular attention in order normalize the situation, which should be checked by future investigative steps

    Efficacy and Effectiveness of Physical Agent Modalities in Complex Regional Pain Syndrome Type I: A Scoping Review

    Complex regional pain syndrome type I (CRPS-I) is a rare condition with huge variability in triggering factors and clinical scenarios. The complexity of the pathophysiology of this condition fosters the proposal of several therapeutic options with different mechanisms of action in both research and clinical practice. An interdisciplinary and multimodal approach, including pharmacological and non-pharmacological interventions, particularly physical therapy, is recommended by international guidelines, but the benefits and harms of available interventions are poorly known. In this scoping review, the clinical rationale for use of physical agent modalities for patients with CRPS-I will be presented. We found 10 studies addressing the role of electromagnetic field therapy, electrotherapy, and laser therapy. Our findings suggest that physical therapy modalities, in particular transcutaneous electrical nerve stimulation (TENS) and pulsed electromagnetic field therapy (PEMF), may contribute to reduce pain and improve function in patients with CRPS-1

    Hepatitis a epidemic in men who have sex with men (MSM) in milan, Italy

    Background and aim of the work: Hepatitis A is an infectious disease characterized by fecal-oral transmission; however, a rise in sexually-transmitted cases has been observed, particularly among \u201cmen who have sex with men\u201d. In Europe, a Hepatitis A epidemic occurred among men who have sex with men between 2016 and 2018. The aim of this study is to describe this Hepatitis A epidemic in the city of Milan and to analyze the incidence of Sexually Transmitted Diseases co-infection among Hepatitis A cases. Methods: Hepatitis A cases were traced and identified. Epidemiological data were collected and Hepatitis A vaccination was investigated. Cases were georeferenced, calculating incidence rates for each Milan Municipality. Viral genotypic analysis was carried out. Results: 353 cases were reported in Milan. Incidence rates resulted significantly higher in males (RR 18.1 CI9 5% 11.5 - 28.4). 70 cases reported foreign travel. 172 cases reported \u201cMen who have Sex with Men\u201d behaviour. Genotypic analysis revealed correlation with strains of the European \u201cMen who have Sex with Men\u201d epidemic. Georeferencing showed asymmetric case distribution. Only 12 cases reported Hepatitis A vaccination. The Relative Risk for syphilis infection among Hepatitis A cases was 133.9 (95% CI 81.7 - 219.7) and 29.7 (95%CI 9.5 - 92.7) for gonorrhea. Conclusions: Most genotyped cases (93.7%) correlated to the European Hepatitis A epidemic among Men who have Sex with Men. Georeferencing showed a greater incidence of Hepatitis A cases in areas characterized by the homosexual community. The higher incidence of Sexually Transmitted Diseases co-infection in Hepatitis A cases correlated to clusters responsible for the European Hepatitis A epidemic, suggests increased sexual promiscuity among Men who have Sex with Men. These data support the need for Hepatitis A vaccination programs and sensitization of Men who have Sex with Men to the adoption of safe sexual practices

    Glider data collected during the Algerian Basin Circulation Unmanned Survey

    We present data collected in the framework of the Algerian BAsin Circulation Unmanned Survey (ABACUS) project. The main objective of ABACUS is the monitoring of the basin circulation and of the surface and intermediate water masses' physical and biological properties in a key region of the Mediterranean Sea circulation. Data presented here have been collected through deep glider cruises in the Western Mediterranean Sea during the autumns of 2014, 2015 and 2016; activities at sea are expected to be repeated during the coming years, so that the dataset will be extended. Glider missions were conducted in the Algerian Basin, between the island of Mallorca and the Algerian coast. Across the three glider missions, eight repeated transects were completed which enabled us to investigate the basin-scale circulation and the presence of mesoscale structures, utilising both the adaptive sampling capabilities of the gliders and the higher resolution of the data. After collection, all data passed a quality control procedure and were then made available through an unrestricted repository host by the SOCIB Data Centre at https://doi.org/10.25704/b200-3vf5. The actual dataset spans three autumn seasons, providing an important contribution to the data collection in the chronically undersampled Algerian Basin. Temperature and salinity data collected in the first 975&thinsp;m of the water column allowed us to identify the main water masses and describe their characteristics, improving the understanding of the dynamics of the region. On the timescale of the project, data show a large variability in the surface layer and reduced variability in the intermediate and deep layers. Our measurements have been successfully compared to data previously collected in the area from 1909 to 2011. Results showed similar overall distribution, ranges and variability to the historical data, with no outliers in the surface or deep layers.</p

    Longitudinal analysis of HIV-1 BF1 recombinant strains in vertically infected children from Argentina reveals a decrease in CRF12_BF pol gene mosaic patterns and high diversity of BF unique recombinant forms

    The HIV-1 epidemic associated to BF1 recombinants in South America is both complex and intriguing, with an underestimated diversity of recombinant structures. Our aim was to explore the characteristics and temporal dynamics of the HIV-1 BF1 epidemic in Argentina, through the study of 172 HIV-1 pol BF1 recombinant sequences obtained from HIV-1 vertically infected patients born from 1986 to 2008. Recombination patterns were characterized by bootscanning, subtype signature analysis, and phylogenetic approaches. Proportion of sequences sharing common ancestry and recombination breakpoints with the Circulating Recombinant Form (CRF) CRF12_BF was compared against sequences with a non-CRF12_BF pattern in three study periods, and by fitting the data to a logistic model. Twenty-eight HIV-1 pol BF1 mosaic structures were identified, including four of the seven South-American CRF_BF-like patterns. However, common ancestry of these sequences with reference CRF strains only confirmed the presence of CRF12_BF (51.1%) and CRF17_BF (1.2%) among the Argentine BF pol sequences. Most non-CRF_BF-like recombinant patterns shared at least one common recombination breakpoint with CRF12_BF. The number of transmissions caused by CRF12_BF viruses decreased in a linear way over time, from 69% in the period 1986–1993 to 46% in 2001–2008. In conclusion, the diversity of HIV-1 pol BF1 recombinant structures in Argentina is much more complex than previously described, with at least two CRFs_BF and 26 BF1 unique recombinant forms. For the first time, we provide evidence of a decrease in the proportion of CRF12_BF viruses transmitted from mother-to-child since the start of the epidemic to the present time in Argentina.Fil: Aulicino, Paula.     Phylodynamics of HIV-1 Circulating Recombinant Forms 12_BF and 38_BF in Argentina and Uruguay

    <p>Abstract</p> <p>Background</p> <p>Although HIV-1 CRF12_BF and CRF38_BF are two epidemiologically important recombinant lineages circulating in Argentina and Uruguay, little is known about their population dynamics.</p> <p>Methods</p> <p>A total of 120 "CRF12_BF-like" and 20 "CRF38_BF-like" <it>pol </it>recombinant sequences collected in Argentina and Uruguay from 1997 to 2009 were subjected to phylogenetic and Bayesian coalescent-based analyses to estimate evolutionary and demographic parameters.</p> <p>Results</p> <p>Phylogenetic analyses revealed that CRF12_BF viruses from Argentina and Uruguay constitute a single epidemic with multiple genetic exchanges among countries; whereas circulation of the CRF38_BF seems to be confined to Uruguay. The mean estimated substitution rate of CRF12_BF at <it>pol </it>gene (2.5 × 10-3 substitutions/site/year) was similar to that previously described for subtype B. According to our estimates, CRF12_BF and CRF38_BF originated at 1983 (1978-1988) and 1986 (1981-1990), respectively. After their emergence, the CRF12_BF and CRF38_BF epidemics seem to have experienced a period of rapid expansion with initial growth rates of around 1.2 year<sup>-1 </sup>and 0.9 year<sup>-1</sup>, respectively. Later, the rate of spread of these CRFs_BF seems to have slowed down since the mid-1990s.</p> <p>Conclusions</p> <p>Our results suggest that CRF12_BF and CRF38_BF viruses were generated during the 1980s, shortly after the estimated introduction of subtype F1 in South America (~1975-1980). After an initial phase of fast exponential expansion, the rate of spread of both CRFs_BF epidemics seems to have slowed down, thereby following a demographic pattern that resembles those previously reported for the HIV-1 epidemics in Brazil, USA, and Western Europe.</p

    Satellite observations for detecting and forecasting sea-ice conditions: A summary of advances made in the SPICES Project by the EU's Horizon 2020 Programme

    The detection, monitoring, and forecasting of sea-ice conditions, including their extremes, is very important for ship navigation and offshore activities, and for monitoring of sea-ice processes and trends. We summarize here recent advances in the monitoring of sea-ice conditions and their extremes from satellite data as well as the development of sea-ice seasonal forecasting capabilities. Our results are the outcome of the three-year (2015-2018) SPICES (Space-borne Observations for Detecting and Forecasting Sea-Ice Cover Extremes) project funded by the EU's Horizon 2020 programme. New SPICES sea-ice products include pancake ice thickness and degree of ice ridging based on synthetic aperture radar imagery, Arctic sea-ice volume and export derived from multisensor satellite data, and melt pond fraction and sea-ice concentration using Soil Moisture and Ocean Salinity (SMOS) radiometer data. Forecasts of July sea-ice conditions from initial conditions in May showed substantial improvement in some Arctic regions after adding sea-ice thickness (SIT) data to the model initialization. The SIT initialization also improved seasonal forecasts for years with extremely low summer sea-ice extent. New SPICES sea-ice products have a demonstrable level of maturity, and with a reasonable amount of further work they can be integrated into various operational sea-ice services

    Large community-acquired Legionnaires&#8217; disease outbreak caused by Legionella pneumophila serogroup 1, Italy, July to August 2018

    In July 2018, a large outbreak of Legionnaires\u2019 disease (LD) caused by Legionella pneumophila serogroup 1 (Lp1) occurred in Bresso, Italy. Fifty-two cases were diagnosed, including five deaths. We performed an epidemiological investigation and prepared a map of the places cases visited during the incubation period. All sites identified as potential sources were investigated and sampled. Association between heavy rainfall and LD cases was evaluated in a case-crossover study. We also performed a case\u2013control study and an aerosol dispersion investigation model. Lp1 was isolated from 22 of 598 analysed water samples; four clinical isolates were typed using monoclonal antibodies and sequence-based typing. Four Lp1 human strains were ST23, of which two were Philadelphia and two were France-Allentown subgroup. Lp1 ST23 France-Allentown was isolated only from a public fountain. In the case-crossover study, extreme precipitation 5\u20136 days before symptom onset was associated with increased LD risk. The aerosol dispersion model showed that the fountain matched the case distribution best. The case\u2013control study demonstrated a significant eightfold increase in risk for cases residing near the public fountain. The three studies and the matching of clinical and environmental Lp1 strains identified the fountain as the source responsible for the epidemic

    Analysis of the Origin and Evolutionary History of HIV-1 CRF28_BF and CRF29_BF Reveals a Decreasing Prevalence in the AIDS Epidemic of Brazil

    HIV-1 subtype B and subtype F are prevalent in the AIDS epidemic of Brazil. Recombinations between these subtypes have generated at least four BF circulating recombinant forms (CRFs). CRF28_BF and CRF29_BF are among the first two BF recombinants being identified in Brazil and they contributed significantly to the epidemic. However, the evolution and demographic histories of the CRFs are unclear.A collection of gag and pol sequences sampled within Brazil was screened for CRF28_BF-like and CRF29_BF-like recombination patterns. A Bayesian coalescent framework was employed to delineate the phylogenetic, divergence time and population dynamics of the virus having CRF28_BF-like and CRF29_BF-like genotype. These recombinants were phylogenetically related to each other and formed a well-supported monophyletic clade dated to 1988-1989. The effective number of infections by these recombinants grew exponentially over a five-year period after their emergence, but then decreased toward the present following a logistic model of population growth. The demographic pattern of both recombinants closely resembles those previously reported for CRF31_BC.We revealed that HIV-1 recombinants of the CRF28_BF/CRF29_BF clade are still circulating in the Brazilian population. These recombinants did not exhibit a strong founder effect and showed a decreasing prevalence in the AIDS epidemic of Brazil. Our data suggested that multiple URFs may also play a role in shaping the epidemic of recombinant BF HIV-1 in the region
